A Little Preparation Goes a Long Way

Please take a few minutes to complete your online patient history form before your appointment. Be sure to let us know about any vision concerns, eye health goals, or questions you'd like to discuss during your visit. If you're unable to complete the form online, we kindly ask that you arrive at least 15 minutes before your scheduled appointment to complete your paperwork in the office.

Helping Us Care for Every Patient

Appointment & Cancellation Policy

We respect your time just as much as we value the opportunity to care for your eyes. Your appointment time is reserved specifically for you. If you need to cancel or reschedule your visit, we kindly ask that you provide at least 24 hours' notice whenever possible. 

Appointments that are canceled with less than 24 hours' notice or missed without notice (no-show) may require a $50 deposit before scheduling a future appointment. This deposit will be applied toward your next visit. We understand that unexpected situations can arise and appreciate your communication with our team.
